OnTrack to Health

Specifically designed for patients and care teams, OnTrack to Health makes following a complex medical regimen less burdensome and more manageable. Your iPhone or iPad becomes an interactive reminder and secure communication tool.
After the application is set up by your healthcare provider, OnTrack to Health will:

- Alert you about which medication to take and when

- Provide you secure and direct messaging with your care team, at any time

- Send your healthcare provider a daily assessment of your self-care

- Provide an educational RSS feed from your provider

This application will not function without being connected to your healthcare provider’s OnTrack Care Giver Management Portal. This portal allows your provider to monitor your progress staying OnTrack to Health.
